Both pilots sound like fuckups. Captain was "too tired" to report it until the CVR was overwritten. Co was passed over twice for promotion as a weak candidate, lack of situational awareness. You should get to see your surgeon or pilots record beforehand.

three accidents involving 777's have had fatalities:

Yep, primarily at the moment of engine failure. The engine slung a fan blade which could have caused injuries and death had it struck the passenger area. Fortunately it looks like it (or some other part at least) struck the baggage/landing gear bay area instead.
